5.     Working Papers [+Medical]

6.     Proof of Citizenship/Alien Status

7.     Proof of Address

8.     Proof of Family Income [SSI award letter or 2 consecutive pay stubs].  

9.     SYEP Application

10.  Proof of Selective Service if you are MALE and 18 YEARS or older http://www.sss.gov/default.htm
